  1. First subscribe to the site.

  2. Click the download button at the top right corner of the pdf document below.

  3. Navigate to the downloaded file on your computer and open it with Adobe Acrobat.

  5. When your application is processed and approved your subscription to the site will be made active, and you will receive an email to Login.
